Louisa County Recorder - Passports

PASSPORT REQUIREMENTS:

FIRST TIME APPLICANTS:
1. Proof of U.S. citizenship
2. Proof of Identity
3. Two recent, identical, color photographs

APPLICANTS-PREVIOUS U.S. PASSPORT:
1. Your most recent passport issued less than 15 years ago and over age 16 when issued.
2. Use Form DS-82 (mail in application-check eligibility on web site, listed below)

CHILDREN:
Children under age 16, both parents or legal guardian must appear with:
1. Evidence of child's U.S. citizenship
2. Evidence of child's relationship to parents/guardian(s)
3. Parent/guardian identification
IF only one parent appears you must also submit one of the following:
1. Second parent's written statement consenting to passport issuance for the child
2. Primary evidence of sole authority to apply, OR
3. A written statement (made under penalty of perjury) explaining the second
parent's unavailability.

COST:
$100.00 for age 16 and over
$85.00 for age under 16
