	General Assistance provides emergency assistance for the needy and poor in Louisa County. If criteria is met the assistance that can be provided is: food, rent, shelter, emergency medical, transportation, utilities and burials. The Community Services office refers the individuals to resources available to them. Application Form Release of Information Notice of Privacy Practices Case Management provides Targeted Case Management Medicaid eligible adults, for individuals with Chronic Mental Illness, Mental Retardation, and Developmental Disabilities. Targeted Case Management Services are provided for Medicaid eligible children with diagnosed Mental Retardation, on the Home and Community Based MR/Waiver. Targeted Case management coordinates and monitors services chosen by the individual to assist them in achieving goals to maintain living in the least restrictive setting possible.
