CASE MANAGER
Louisa County is seeking an experienced individual to provide Medicaid case management services to 
persons with mental retardation, chronic mental illness and developmental disabilities.  The position 
requires a bachelor degree with a major, or at least 30 semester hours, in the behavioral sciences, 
education, health care, human services administration, or the social sciences; and a minimum of one 
year of experience in the delivery of services to adults and children with mental retardation, mental 
illness, and developmental disabilities; or an Iowa license to practice as a registered nurse and at 
least 3 years of experience in the delivery of services to adults and children with mental retardation, 
mental illness and developmental disabilities.  Bilingual re: English and Spanish helpful.  This is a 
part-time position (20 hrs/wk) within Louisa County Community Services.

General Assistance provides emergency assistance for the needy and poor in Louisa County. If criteria is met the assistance that can be provided is: food, rent, shelter, emergency medical, transportation, utilities and burials. The Community Services office refers the individuals to resources available to them.

Case Management provides Targeted Case Management Medicaid eligible adults, for individuals with Chronic Mental Illness, Mental Retardation, and Developmental Disabilities. Targeted Case Management Services are provided for Medicaid eligible children with diagnosed Mental Retardation, on the Home and Community Based MR/Waiver. Targeted Case management coordinates and monitors services chosen by the individual to assist them in achieving goals to maintain living in the least restrictive setting possible.
